Supported Independent Living (SIL)
We offer Supported Independent Living (SIL) which supports our clients with disability to live holistic, healthy and connected lives in an accommodation setting that best meets the client's needs. Our dedicated SIL employees provide assistance with activities of daily living.
Personal Care
Our exceptional service is what makes us better from all the other home care centres. All of our home care team get comprehensive training before giving assistance to our clients. Our standards are that we only hire care providers who we would have hired for our own loved ones.
Activity and Daily Living
When we provide a service, we consult with the most important person - the client. Our tailored and support services encourage independence and guide them to make choices in daily life and to do day-to-day activities. We adopt a person-centred approach support plan to fulfil their needs and wellbeing. With our program we thrive to enhance their confidence, independence, life choices, and self-esteem.
Community Access
Independence matters a lot to us. We know very well how significant it is for clients to feel responsible for their world since they understand their needs and objectives better than anybody. Be assured our community access team understands the need of clients choice and control.
Community Nursing
SSM’s registered nurses are trained to help, either it be recovery at home after undergoing a medical event or managing an ongoing condition. We tailor our professional services around your personal requirements giving you freedom to recover and recoup without any stress. Our customer care team first learn about your specific healthcare needs and then offer solutions to provide you the care that best suits your needs and lifestyle.
Transport / Vehicle Modification
If you have a disability, it may affect your ability to travel in a car. There are a range of modifications that can be made to a car to enable a person to safely drive or travel as a passenger. Vehicle modifications range from minor or major. Synergy Support and Motivation have you covered. Your need, our commitment.
